Cumulative evidence in psychotherapy suggests that we not only do not improve with experience, our typical professional development efforts by attending workshops, and engaging in clinical supervision do not lead to better performance.<\/p><\/blockquote><script async src=\"\/\/cdn.embedly.com\/widgets\/platform.js\" charset=\"UTF-8\"><\/script>\n<\/div><\/figure>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">A Step Forward\u2026 Into Overwhelm.<\/h2>\n\n\n\n<p class=\"has-medium-font-size\">But from everything I was learning so far, those questions I was asking myself were not good questions to begin with. Taking all into account, the existing evidence would suggest that I was&nbsp;<a href=\"https:\/\/darylchow.com\/frontiers\/signs-that-therapists-are-barking-up-the-wrong-tree-in-our-professional-development\/\">barking up the wrong tree<\/a>.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">I have a tendency to obsess with the&nbsp;<strong>HOW<\/strong>&nbsp;before I address the&nbsp;<strong>What<\/strong>.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">What in the world should I be working on?!<\/p>\n\n\n\n<p class=\"has-medium-font-size\">Moreover, unlike other fields like chess, sports, and classical music that I was learning about deliberate practice, psychotherapy was somewhat different. The environment was not so \u201ckind\u201d (i.e., the feedback loop for the target of learning and the impact of performance are clear). Psychotherapy seemed to fall in a less kind environment, or what Robin Hogarth would call, Wicked Environment (i.e., the rules of the context are often unclear, and the feedback loop for the target of learning and the impact on the performances are delayed, inaccurate, or both. It was&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Bloom's_taxonomy\">Benjamin Bloom\u2019s Taxonomy<\/a>, a framework through which teachers use to guide their students through the cognitive learning process.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/substackcdn.com\/image\/fetch\/f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2F2458402b-12b2-4827-9d27-7440dfafbd40_1500x1109.jpeg\" target=\"_blank\" rel=\"noreferrer noopener\"><img decoding=\"async\" src=\"https:\/\/substackcdn.com\/image\/fetch\/w_1456,c_limit,f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2F2458402b-12b2-4827-9d27-7440dfafbd40_1500x1109.jpeg\" alt=\"Picturing deeper learning with Bloom\u2019s Taxonomy\" title=\"Picturing deeper learning with Bloom\u2019s Taxonomy\"\/><\/a><figcaption class=\"wp-element-caption\">Rawia Inaim \/&nbsp;Kwantlen Polytechnic University<\/figcaption><\/figure>\n\n\n\n<p class=\"has-medium-font-size\">So I thought,&nbsp;<em>Easy, I should do something like this for the therapy and simply break it down into parts, so that I can work on it.<\/em><\/p>\n\n\n\n<p class=\"has-medium-font-size\">I didn\u2019t expect that I would become somewhat snowed under with how to even begin doing this.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">One dimension I took was to break the therapy hour down into chunks.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Beginnings:<\/h3>\n\n\n\n<p class=\"has-medium-font-size\"><em>How do you begin a first session? How do you begin a subsequent session? How do you induct someone into therapy?<\/em><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Middles:<\/h3>\n\n\n\n<p class=\"has-medium-font-size\"><em>How do you deepen the process?<\/em><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><em>How do you tap and utilise client factors, their resources, their Why\u2019s?<\/em><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Endings<\/h3>\n\n\n\n<p class=\"has-medium-font-size\"><em>How do you close a session?<\/em><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><em>How do you create an impactful closing?<\/em><\/p>\n\n\n\n<p class=\"has-medium-font-size\"><em>How do you gather feedback to feed-forward into future sessions?<\/em><\/p>\n\n\n\n<p class=\"has-medium-font-size\">This to me, was a good start. But something was missing. This felt flat, lifeless, and two-dimensional. I soon realised I need to add the other &#8220;hidden\u201d dimension in the therapy process. The stuff that is \u201cfelt, not seen\u201d within the hour of deep conversation, e.g., attunement, emotional vibrancy, dealing with minor and major mis-alignments, the mobilisation of the will of the client, the use of the self, etc.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">I listed all these down.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">I didn\u2019t expect that it would cast even more doubt about my ability as a therapist.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">I later learned that this was actually to be expected. I chanced upon a study by Rozenbilt and Keil (2002) called,&nbsp;<em><strong><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1207\/s15516709cog2605_1\">The Misunderstood Limits of Folk Science: An Illusion of Explanatory Depth.<\/a><\/strong><\/em><\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/substackcdn.com\/image\/fetch\/f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2F7729447a-040c-4af5-90a2-684736b72603_1424x1362.png\" target=\"_blank\" rel=\"noreferrer noopener\"><img decoding=\"async\" src=\"https:\/\/substackcdn.com\/image\/fetch\/w_1456,c_limit,f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2F7729447a-040c-4af5-90a2-684736b72603_1424x1362.png\" alt=\"\"\/><\/a><figcaption class=\"wp-element-caption\"><em><strong>Illusion of Explanatory Depth (<\/strong><\/em><strong>IOED)<\/strong><\/figcaption><\/figure>\n\n\n\n<p class=\"has-medium-font-size\">To get a sense of what the&nbsp;<em><strong>Illusion of Explanatory Depth or&nbsp;<\/strong><\/em>IOED is about, here is a thought experiment:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li class=\"has-medium-font-size\"><strong>How well do you know how a toilet flush works?<\/strong>\u00a0Rate that from a 0 to 10. Now, take a piece of paper and explain it through step-by-step, how\u00a0<em>does<\/em>\u00a0a toilet flush work. Again, rate this from a 0 to 10.<\/li>\n\n\n\n<li class=\"has-medium-font-size\"><strong>How well do you know how a bicycle works?<\/strong>\u00a0Rate that from a 0 to 10. Now, take a piece of paper and explain it through step-by-step, how\u00a0<em>does<\/em>\u00a0a bicycle work. Again, rate this from a 0 to 10.<\/li>\n<\/ol>\n\n\n\n<p class=\"has-medium-font-size\">Chances are, your rating is likely to dip.&nbsp;<\/p>\n\n\n\n<p class=\"has-medium-font-size\">This, is the IOED.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Who Not How<\/h3>\n\n\n\n<p class=\"has-medium-font-size\">I didn\u2019t know how to proceed further. I had fell into IOED. It felt like a pointless hole to fall into. Who would even give a damn about this?<\/p>\n\n\n\n<p class=\"has-medium-font-size\">This was when Scott Miller offered me some perspective hit the nail in the head. The Debate on Common Factors (Part III) \u2b55\ufe0f<\/a><a href=\"https:\/\/darylchow.substack.com\/p\/frontiers-friday-135-the-debate-on\"><\/a><\/h4>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p class=\"has-medium-font-size\">We began to collaborate and flesh out a better version of this, crossed-map to what others like Bruce Wampold have found through the past few decades of psychotherapy research.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">If Scott wasn\u2019t in the picture, I doubt the TDPA would have seen the light of day. It would probably be just a little document\/spreadsheet existing in my PC.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">This taught me something: Everyone needs a WHO.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">Scott was the WHO before we could take it further to where it is today (at version 6).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Chase Two Rabbits, and You\u2019d Catch None.<\/h2>\n\n\n\n<p class=\"has-medium-font-size\">We got underway to develop and refine the TDPA worksheet.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">Frankly, Scott had more faith of its utility than I did. I thought to myself, he must be seeing something that I don\u2019t.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">The next road-bump was somewhat embarrassing. On 10th of November, 2015, I had examined my outcomes, revisited my caseload, and completed my own TDPA. Then I narrowed down to 3 THINGS to keep focused on and improve:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li class=\"has-medium-font-size\">Elicit nuanced feedback<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Conveying explicit care<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Suggestion for homework\/tasks between sessions<\/li>\n<\/ol>\n\n\n\n<p class=\"has-medium-font-size\">Looking back at my baseline performance, I was pretty happy about this plan.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">Three weeks later, I had completely forgotten about what those 3 THINGS were. Ugh.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/substackcdn.com\/image\/fetch\/f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2Fb60c2504-cef8-4ea6-a4f8-120a5ff5e63a_1960x1130.png\" target=\"_blank\" rel=\"noreferrer noopener\"><img decoding=\"async\" src=\"https:\/\/substackcdn.com\/image\/fetch\/w_1456,c_limit,f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2Fb60c2504-cef8-4ea6-a4f8-120a5ff5e63a_1960x1130.png\" alt=\"\"\/><\/a><figcaption class=\"wp-element-caption\">A screenshot of my TDPA from November 2015.<\/figcaption><\/figure>\n\n\n\n<p class=\"has-medium-font-size\">Meanwhile, Scott and I were running our annual Deliberate Practice intensive workshops in Chicago. We learned others had similar experiences of losing track of the focus, or feeling overwhelmed. While working on three things seemed manageable, in reality, it was still too many, when you consider the multi-faceted lives many of us have.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">On 25th of January, 2016, I conceded and noted to myself,<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"has-medium-font-size\"><em>\u201cVery hard to focus even on just three things! Need to narrow it down to just one.\u201d<\/em><\/p>\n<\/blockquote>\n\n\n\n<p class=\"has-medium-font-size\">This was a small but significant insight. I have to focus on ONE thing, not two, and certainly not three.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">I recalled the saying,&nbsp;<strong>Chase two rabbits and you catch none.<\/strong><\/p>\n\n\n\n<p class=\"has-medium-font-size\">It is humbling to be struck by the law of diminishing returns, which is probably as real as the law of gravity. The law of diminishing returns is that the more goals you have, the less you are able to achieve it.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">My One Thing Over Time<\/h3>\n\n\n\n<p class=\"has-medium-font-size\">Over time, as I meandered through and continued my clinical practice and sought guidance from supervisors and books, here wad the ONE thing I was focusing on at different stages:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-medium-font-size\">12th of Apr\u201916: Tap and utilise\u00a0<strong>client resources<\/strong>\u00a0to heighten engagement.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">7th of Aug\u201916: Re-contact supervisor as I was losing track of where I was going.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">27th of Feb\u201917: Re-devote to once a week reviewing specific parts of therapy recordings, plus get supervisor to watch.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">27th of Sept\u201917:\u00a0<strong>Consolidate<\/strong>\u00a0what I\u2019ve learned so far. I also began to incorporate the idea of \u201c<strong>Anti-Goals<\/strong>\u201d at this stage, i.e.,\u00a0<strong>What\u00a0<\/strong><em><strong>not<\/strong><\/em><strong>\u00a0to focus on<\/strong>. This is to curb any short-term overly-caffeinated optimism I might have to work on more.<a href=\"https:\/\/darylchow.substack.com\/p\/ff185#footnote-1-144493800\">1<\/a><\/li>\n\n\n\n<li class=\"has-medium-font-size\">22nd of May\u201918:\u00a0<strong>To be less formal; more personable<\/strong>. (Looking back, this was a significant thing for me, as I found myself somewhat unwittingly becoming cold and clinical in-sessions).<\/li>\n\n\n\n<li class=\"has-medium-font-size\">24th of July\u201918: Balance between being strategic\/coachy and emotive.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">24th of Oct\u201918: Same thing. Balance between being strategic\/coachy and emotive.<\/li>\n\n\n\n<li class=\"has-medium-font-size\">26th of Feb\u201d19:\u00a0<strong>Mobilise the will<\/strong>\u00a0of the client before going further. (Otherwise, I become the one doing all the heavy-lifting in sessions and still not making progress)\u2026 (Flash forward)<\/li>\n\n\n\n<li class=\"has-medium-font-size\">Current: To allow myself to be more expressive and less inhibitive in-session.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">What is the Big Deal Anyway?<\/h2>\n\n\n\n<p class=\"has-medium-font-size\">The TDPA is not going to bring world peace.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">Neither will it answer&nbsp;<em>all<\/em>&nbsp;of your professional development woes. In fact, as you can tell by now, the TDPA might first inundate you, cast shadows of doubt upon you, and if you see it through, it will provide a light forward, for the rest of your professional career.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">The TDPA serves as somewhat of an antidote the problem in our field. I have previously addressed some of the pressing issues at hand in our field that has relevance to the existence of the TDPA. In gist, there are inherent problems in the way we train, supervise, and perceive about the influence of clinical experience.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">Read this:<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/darylchow.com\/frontiers\/what-are-the-most-pressing-problems-in-the-practice-of-psychotherapy\/\" target=\"_blank\" rel=\"noreferrer noopener\"><img decoding=\"async\" src=\"https:\/\/substackcdn.com\/image\/fetch\/w_1456,c_limit,f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2F0d08ea63-59da-4687-8fca-68bed43eda9d_289x420.png\" alt=\"\"\/><\/a><\/figure>\n\n\n\n<p class=\"has-medium-font-size\">As we cumulate clinical experience, one of the challenges we face is to overcome automaticity, or sometimes referred to as The \u201cOK\u201d plateau.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/substackcdn.com\/image\/fetch\/f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2Fa8692005-c1c0-43f5-bc3f-392f892c2c80_1024x1024.png\" target=\"_blank\" rel=\"noreferrer noopener\"><img decoding=\"async\" src=\"https:\/\/substackcdn.com\/image\/fetch\/w_1456,c_limit,f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2Fa8692005-c1c0-43f5-bc3f-392f892c2c80_1024x1024.png\" alt=\"\" title=\"\"\/><\/a><\/figure>\n\n\n\n<p class=\"has-medium-font-size\">Our hope is that the TDPA helps you counteract the \u201cOK\u201d Plateau. You will likely become conscious about what you are doing. But the aim is not to stay conscious about it, it is part of the cycle to then move what you\u2019ve acquired into the unconscious.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/substackcdn.com\/image\/fetch\/f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2Fbee1dd69-0c98-4a78-a1c6-ec53e4a98cfe_1024x1024.png\" target=\"_blank\" rel=\"noreferrer noopener\"><img decoding=\"async\" src=\"https:\/\/substackcdn.com\/image\/fetch\/w_1456,c_limit,f_auto,q_auto:good,fl_progressive:steep\/https%3A%2F%2Fsubstack-post-media.s3.amazonaws.com%2Fpublic%2Fimages%2Fbee1dd69-0c98-4a78-a1c6-ec53e4a98cfe_1024x1024.png\" alt=\"\" title=\"\"\/><\/a><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">The Work Before the Work<\/h2>\n\n\n\n<p class=\"has-medium-font-size\">The night before I visited my dentist, I flossed my teeth, twice.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">What we do before the work, &#8220;<strong><a href=\"https:\/\/darylchow.com\/frontiers\/the-work-before-the-work\/\">the work before the work<\/a><\/strong>&#8221; matters a great deal.<\/p>\n\n\n\n<p class=\"has-medium-font-size\">It has once been attributed to Lincoln who said,<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"has-medium-font-size\"><strong>If I had eight hours to chop down a tree, I&#8217;ll spend six hours sharpening the saw.<\/strong><\/p>\n<\/blockquote>\n\n\n\n<p class=\"has-medium-font-size\">Take the thing to figure out&nbsp;<strong>WHAT<\/strong>&nbsp;to work on before obsessing about the HOW.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<p class=\"has-text-align-center has-medium-font-size\"><strong><em>If you would like to learn more topics that can help your professional development, subscribe to the Frontiers of Psychotherapist Development (FPD).
