Having More Than ONE Area to Focus on Improving<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Truth is, most therapists are pushed beyond the brink. On top of clinical practice, administrative duties, and fueled by a desire to read the latest and the greatest research publications\u2014oh, how can I forget that we have a personal and family life\u2014 having more than one area to work on creates nothing short of overwhelming.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Feeling overwhelmed is not necessarily a function of having too much to do but rather not knowing what to do next.[1]<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">In the early stages of the development of the Taxonomy of Deliberate Practice Activities (TDPA), Scott Miller and I figured that therapists can focus on improving three of the identified from the list. After some months of working on it as a practitioner<\/span>, <span style=\"font-family: 'Book Antiqua', Palatino;\">I realised<\/span> tha<span style=\"font-family: 'Book Antiqua', Palatino;\">t even 2-3 areas to work on <\/span>is<span style=\"font-family: 'Book Antiqua', Palatino;\"> OVERWHELMING. We began to get some feedback from others from the deliberate practice workshops that we run annually in Chicago (come join us this <a href=\"https:\/\/www.eventbrite.ie\/e\/deliberate-practice-intensive-2018-tickets-38819192249\" rel=\"noopener\"><span style=\"color: #3366ff;\">Aug&#8217;18<\/span><\/a>!). I was not alone in this experience.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">In the latest version of the taxonomy, after the ranking of the top 3 activities, we recommend drilling it down to ONE area to work on.[2]<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">2. Not Having ONE Specific Area to Focus on Improving<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">&#8220;Improve engagement,&#8221; is to vague a goal.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">Sometimes the goal is to figure out the goal. Get focused on defining the goals, and get less wedded to solutions.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">Let&#8217;s think this through<\/span>:<span style=\"font-family: 'Book Antiqua', Palatino;\"> What consists of a good engagement? Goal consensus, emotional bond, agreement on approach and method?[3] What else? What aspects of the human interaction can facilitate resonance?<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">(See related <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/02\/15\/mastery-learning-for-therapists-figure-out-the-what-before-the-how\/\"><span style=\"color: #3366ff;\">post<\/span><\/a><span style=\"color: #3366ff;\">.<\/span>)<\/span><\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">3. Not Having A System of Practice<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Once we&#8217;ve defined the ONE area to work on, we need a system to help us sustain this through our busy lives.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">In order for deliberate practice to happen, we need a <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/12\/08\/the-biggest-stumbling-block-towards-individualised-professional-development\/\"><span style=\"color: #3366ff;\">system of practice<\/span><\/a><span style=\"color: #3366ff;\">.<\/span>\u00a0<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">We can\u2019t just rely on willpower; our energy needs to be channeled to clinical practice and deliberate practice, not \u201cwhen to practice\u201d. Instead, we need to rely on a system of practice, and **automating** this process so that we do not need to be thinking, &#8220;ok, when I get some free time,&#8221; or \u201cOk, let\u2019s see if a client cancels today, and I\u2019d slip in some deliberate practice.\u201d<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">4. Trying to &#8220;Do Deliberate Practice&#8221; in a Particular Method<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">This point often surprises people I coach. Focusing on specific methods has little to no leverage on actually improving your outcomes; let alone focusing on a <em>specific<\/em> feature of a <em>specific<\/em> method.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Do not turn deliberate practice into a noun. Instead, deliberate practice is a framework for engaging in focused and deep work, aimed at getting better at achieving better results with your clients.<\/span><\/p>\n<p>Gleaning from the study of expertise and expert performance, the following are the four tenets of deliberate practice:<\/p>\n<p><img decoding=\"async\" class=\"aligncenter size-medium wp-image-1629\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2018\/01\/Screen-Shot-2018-01-20-at-3.07.36-PM-300x292.png\" alt=\"\" width=\"300\" height=\"292\" srcset=\"https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2018\/01\/Screen-Shot-2018-01-20-at-3.07.36-PM-300x292.png 300w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2018\/01\/Screen-Shot-2018-01-20-at-3.07.36-PM.png 360w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">See our recent edited hapters for an elaboration on how these four tenents apply to our efforts in deliberate practice:<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino; font-size: 12pt;\">Chow, D. (See the series of <\/span>blogposts<span style=\"font-family: 'Book Antiqua', Palatino;\"> on first principles,<span style=\"color: #3366ff;\"><a href=\"http:\/\/darylchow.com\/frontiers\/2017\/10\/27\/develop-first-principles-before-the-methods\/\"> I<\/a>, <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/11\/06\/three-ways-to-develop-first-principles-in-your-clinical-practice\/\"><span style=\"color: #3366ff;\">II<\/span><\/a>, <\/span><a href=\"http:\/\/darylchow.com\/frontiers\/2017\/11\/10\/first-principles-the-5-step-process-for-deep-and-accelerated-learning-in-therapy\/\"><span style=\"color: #3366ff;\">III<\/span><\/a>).<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">5. Focusing Exclusively on Content Knowledge<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\"><a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><img decoding=\"async\" class=\"aligncenter wp-image-257\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2016\/02\/3-Types-of-Knowledge.png\" alt=\"\" width=\"323\" height=\"264\" srcset=\"https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2016\/02\/3-Types-of-Knowledge.png 476w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2016\/02\/3-Types-of-Knowledge-300x246.png 300w\" sizes=\"(max-width: 323px) 100vw, 323px\" \/><\/a><\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">Sometimes there is indeed a gap in our knowledge in a particular issue our client presents (i.e., <a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">content knowledge<\/span><\/a>). When that happens, go read up about it.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">However, most issues that therapists face relate to<a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><span style=\"color: #3366ff;\"> <em>process<\/em>\u00a0knowledge<\/span><\/a> and <a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><span style=\"color: #3366ff;\"><em>conditional<\/em><\/span>\u00a0<span style=\"color: #3366ff;\">knowledge<\/span><\/a>.<\/span><\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">6. Focusing on the Numbers<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Early adopters of routine outcome monitoring or feedback informed treatment (FIT) tend to get fixated with the numbers.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">It\u2019s not about the numbers.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">It\u2019s not about the reliable change index or the clinical cutoffs.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">It\u2019s about the story behind the numbers.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">(see related <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/12\/01\/do-not-become-attached-to-the-outcome\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">post<\/span><\/a>)<\/span><\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">7. Emphasis on the self<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">While we are to work on improving our craft, we must not lose sight that the emphasis is not on the egoic self. Instead, the emphasis is to help our clients <\/span>optimise<span style=\"font-family: 'Book Antiqua', Palatino;\"> the chances of experiencing growth, relief from suffering, and benefit from their encounter with you.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Think of it this way: We need more <\/span>outrospection<span style=\"font-family: 'Book Antiqua', Palatino;\"> than introspection. We need to focus more on being helpful than on being good.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">So when you are working to improve your craft, focus on your client.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">(See related <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/11\/24\/the-paradox-of-focusing-on-the-therapist\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">post<\/span><\/a><span style=\"color: #3366ff;\">)<\/span><\/span><\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">8. Not Consulting a Mentor<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">I often asked clinicians and clients alike, \u201cWho has guided you in your life that has created an impact?\u201d It\u2019s such a heartache to hear that many people had no one to turn to for guidance.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Richard Rohr says that too many people are \u201cfather hungry.\u201d<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Contrary to the genre, there is no such thing as \u201cself-help.\u201d Even when we are reading a book for guidance, the author is providing some <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/09\/28\/specific-ways-to-build-a-portfolio-of-mentors\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">instrumental<\/span><\/a> help to the active reader.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Somewhere in our culture (and literature), we are led to believe that when we get older with more experience, we know how to help ourselves in designing a way to improve our work. \u00a0Maybe that&#8217;s why it\u2019s &#8220;lonely at the top.\u201d<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">It\u2019s hard to see what we can\u2019t see.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">9. Detached Professional Development from Actual Client Outcomes<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">We play differently when we are keeping score [4]. Don&#8217;t believe this? Just watch a bunch of kids playing basketball. They play very differently when they are just shooting around, compared to when a game is on and they are keeping scores.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">Attending workshops, trainings, and supervision without an idea on how your clients are doing is like bowling with a curtain covering the pins; you don&#8217;t know if it&#8217;s a strike, a spare, or a complete miss (okay, maybe if you heard the pins, it would mean that at least you&#8217;ve hit a pin).<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">I argue that too much effort, time and money is wasted on CE\/PD activities. It may make us feel good (\u201cI found this course so helpful\u201d), but it does not translate to actual benefit for our clients.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h2><span style=\"font-family: 'Book Antiqua', Palatino;\">10. Supervision That Does Not Translate to Improvement<\/span><\/h2>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">We have less of an excuse for supervision than on other professional development activities.<\/span><br \/>\n<span style=\"font-family: 'Book Antiqua', Palatino;\">Supervision is meant to be individualised, isn\u2019t it?<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">I worry that supervisors spend too much time on <a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">content knowledge<\/span><\/a> and less on <a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">process<\/span><\/a> and <a href=\"http:\/\/darylchow.com\/frontiers\/2016\/02\/12\/three-types-of-knowledge-and-why-this-matters-to-psychotherapy\/\" rel=\"noopener\"><span style=\"color: #3366ff;\">conditional knowledge<\/span><\/a>\u2014and crucially, there\u2019s a lack of staying close to actual client outcomes.<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">(See these two related posts on clinical supervision<span style=\"color: #3366ff;\">:<a href=\"http:\/\/darylchow.com\/frontiers\/2015\/03\/05\/5-the-scandal-of-clinical-supervision-heres-the-shocker-part-1-of-2\/\"> I<\/a> <\/span>&amp;<span style=\"color: #3366ff;\"><a href=\"http:\/\/darylchow.com\/frontiers\/2015\/03\/05\/6-the-scandal-of-clinical-supervision-how-to-resolve-it-part-2-of-2\/\"><span style=\"color: #3366ff;\"> II<\/span><\/a><\/span>)<\/span><\/p>\n<p><span style=\"font-family: 'Book Antiqua', Palatino;\">p\/s: There\u2019s a little more time left.
