{"id":1513,"date":"2017-12-08T03:54:43","date_gmt":"2017-12-07T19:54:43","guid":{"rendered":"http:\/\/darylchow.com\/frontiers\/?p=1513"},"modified":"2019-05-03T16:12:14","modified_gmt":"2019-05-03T08:12:14","slug":"the-biggest-stumbling-block-towards-individualised-professional-development","status":"publish","type":"post","link":"https:\/\/darylchow.com\/frontiers\/the-biggest-stumbling-block-towards-individualised-professional-development\/","title":{"rendered":"The Biggest Stumbling Block Towards Individualised Professional Development"},"content":{"rendered":"<p>The biggest barrier towards your professional development is not on how to get from zero to 10, but from zero to one.[1]<br \/>\nMany therapists struggle with systematically measuring their outcomes.<\/p>\n<p>Because of this, they lack a clear <strong>baseline performance<\/strong>.<\/p>\n<p><!--more--><\/p>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\"aligncenter size-full wp-image-1514\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Do-Not-Become-Attached-to-Outcomes-2.png\" alt=\"\" width=\"560\" height=\"315\" srcset=\"https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Do-Not-Become-Attached-to-Outcomes-2.png 560w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Do-Not-Become-Attached-to-Outcomes-2-300x169.png 300w\" sizes=\"(max-width: 560px) 100vw, 560px\" \/><\/p>\n<p style=\"text-align: right;\"><span style=\"font-family: 'Book Antiqua', Palatino; font-size: 10pt;\"><em>Photo by <a href=\"https:\/\/unsplash.com\/photos\/zX9KPt1Hl_c?utm_source=unsplash&amp;utm_medium=referral&amp;utm_content=creditCopyText\"><span style=\"color: #3366ff;\">Bethany<\/span> <span style=\"color: #3366ff;\">Legg<\/span><\/a><\/em><\/span><\/p>\n<p>While the interest continues to grow for therapists in deliberate practice, it is hard to appreciate the effort and tenacity it takes to consistently measure outcomes, one client at a time, in a systematic and reliable manner.<\/p>\n<p>Even though in this <a href=\"http:\/\/darylchow.com\/frontiers\/\">Frontiers\u2026<\/a> blog, I\u2019ve talked alot about ways for an individualised professional development (we even developed a taxonomy to deal with identifying the activities for therapists to work on[2]), one cannot truly begin the journey without a system of measuring results.<\/p>\n<p>Management guru Peter Drucker points out that what gets measured, get managed.<\/p>\n<p>Without the information of your baseline performance, it\u2019s very easy to <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/03\/06\/signs-that-therapists-are-barking-up-the-wrong-tree-in-our-professional-development\/\">lose track<\/a>. For starters, your assumption of what to work on is typically based on ideology rather than reality.<\/p>\n<h2>Measurement Precedes Professional Development<\/h2>\n<p>In workshops, I repeatedly hear practitioners ask the following question (see also a previous <a href=\"http:\/\/darylchow.com\/frontiers\/2017\/08\/30\/i-get-the-goose-bumps\/\">post<\/a> on this):<\/p>\n<h2>What does improvement mean?<\/h2>\n<p><span style=\"font-family: Helvetica;\"><img decoding=\"async\" class=\"wp-image-1516 alignleft\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Evernote-Snapshot-20171125-171214-copy.jpg\" alt=\"\" width=\"225\" height=\"209\" \/><\/span><\/p>\n<p>Simply, improvement means get better results with your clients compared to your baseline performance.<br \/>\nWithout a baseline information, you would be on a hamster wheel. Traveling without moving. Without a reference point, though with the best of intentions, how would you know what you are doing makes a den of a difference to your overall outcomes?<\/p>\n<h2>&#8220;Am I Getting Any Better?&#8221;<\/h2>\n<h2><span style=\"font-family: Helvetica;\"><img decoding=\"async\" class=\"size-full wp-image-1515 alignright\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Evernote-Snapshot-20171125-1712142-copy.jpg\" alt=\"\" width=\"224\" height=\"207\" \/><\/span><\/h2>\n<p>Finally, one of the biggest source of therapist\u2019s personal frustration and burnout \u2014one that is often left unspoken\u2014 is the unanswered question \u201cAm I get any better?\u201d (This is an important question. Besides, real development is inoculative of <a href=\"https:\/\/www.psychotherapynetworker.org\/magazine\/article\/36\/burnout-reconsidered\">burnout<\/a>).<\/p>\n<p>Recent studies suggest that on average, we do NOT get better over time.[3] Yes, that\u2019s right. <a href=\"http:\/\/darylchow.com\/frontiers\/2015\/01\/17\/3-clinical-practice-vs-deliberate-practice\/\">Experience<\/a> is not predictor of better outcomes. And even though we hail clinical supervision as our \u201csignature pedagogy,\u201d our efforts in this tradition doesn\u2019t help us improve on our outcomes.[4] I know, it\u2019s not the most uplifting news.<\/p>\n<blockquote>\n<p style=\"text-align: center;\"><span style=\"font-size: 18pt;\"><strong><span style=\"font-family: 'Times New Roman', Times;\">What stands in the way of a concerted attempt at routine outcome monitoring, is not because of a lack of will, but a lack of a system.<\/span><\/strong><span style=\"font-family: 'Times New Roman', Times; font-size: 10pt;\">[5]<\/span> <\/span><\/p>\n<p style=\"text-align: center;\"><span style=\"font-size: 18pt;\"><strong><span style=\"font-family: 'Times New Roman', Times;\">We need a <em>system of practice<\/em> in order for deliberate practice to happen.<\/span><\/strong><\/span><\/p>\n<\/blockquote>\n<p>A\u00a0<strong>system of practice<\/strong>\u00a0means:<\/p>\n<h3>1. Pre-commitment<\/h3>\n<p>Pre-decide what measures you are going to use, and use it with EVERY client at EVERY session. Remember, do not simply value what you measure, measure what is of <em>value<\/em>.<\/p>\n<p>As a pre-commitment device[6], if you are starting out employing routine outcome monitoring (ROM) in your practice, suspend any judgement about the utility of the measures, until you have completed at least 20-30 closed cases using outcomes and alliance measures. Once you get to that phase, that is where the fun begins (and the road to a real personalised professional development). You can start to get an idea of your outcomes!<br \/>\nAs a pre-commitment, you must trust the tangible BENEFITS. If you need to be convinced, read the evidence[7]. This not in short supply. Ultimately, committing to measuring your outcomes one-client-at-a-time, requires you to think the long-game.<\/p>\n<h3>2. Make it Easy on Yourself<\/h3>\n<p>Make sure the forms are readily available in your practice desk. Better yet, use some online tool to help you manage this.<br \/>\nCheck out:<\/p>\n<p><a href=\"http:\/\/myoutcomes.com\/\">Myoutcomes.com<\/a><br \/>\n<a href=\"http:\/\/pragmatictracker.com\/\">pragmatictracker.com<\/a><br \/>\n<a href=\"http:\/\/fit-outcomes.com\/\">fit-outcomes.co<\/a>m<br \/>\n<a href=\"https:\/\/openfitapp.com\/\">Open Fit<\/a><\/p>\n<p>Test drive them. <a href=\"http:\/\/myoutcomes.com\/\">Myoutcomes.com<\/a>, <a href=\"http:\/\/fit-outcomes.com\/\">fit-outcomes.com<\/a> and <a href=\"https:\/\/openfitapp.com\/\">Open Fit<\/a> provide the use of the Outcome Rating Scale (ORS) and Session Rating Scale, PLUS algorithms running at the back to compare with trends of \u201csuccessful\u201d and \u201cunsuccessful\u201d cases as you move from session to session. <a href=\"http:\/\/pragmatictracker.com\/\">Pragmatictracker.com<\/a> does not provide such algorithms, but the plus side for PT is that you can <em>customise<\/em> various type of measures you want to use in your clinical practice, alongside the ORS and SRS as well.<\/p>\n<p>(I get asked a lot on what I use. I employ the ORS and SRS at every session, and I combine this with the Clinical Routine Outcome Evaluation-10 at the first, fifth and every five sessions onwards. The CORE-10 gives me quick valuable information that I consider important in my work, especially at the first session, such as risk, intrusive memories, and social support).<\/p>\n<h3>3. Make The Benefits Visible<\/h3>\n<p>Graph out the progress in each session. Make the graph visible to you and your client. Feed this back to your client, and make the information come alive into the flesh and bones of the therapy hour. Use this as a springboard. Let the feedback feed-forward. Use this not just as an assessment tool, but a conversational tool.<\/p>\n<p style=\"text-align: center;\">~~~<\/p>\n<p>Just like the GP\u2019s use of a thermometer, there is nothing magical about the use of feedback tools. The use of feedback measures gives us the <em>attentional<\/em> cue on what to look out for.<\/p>\n<p>Ultimately, remember this:<\/p>\n<blockquote>\n<p style=\"text-align: center;\"><span style=\"font-family: 'Times New Roman', Times; font-size: 18pt;\"><strong>Professional Development = Evaluating + Improvement<\/strong><\/span><\/p>\n<\/blockquote>\n<p>While it\u2019s not the most attractive, we often neglect the <strong>Evaluation<\/strong> component of the equation (Ironically, organisations tend to harp more on evaluation, and much less on helping therapists improve. You can tell when all they are insistent on capturing just the pre-post session outcomes, and not helping practitioners weave this piece of information into the sessions.)<\/p>\n<p>So the question is, do you know how effective you are?
