Similarly, the Stoics point out that we do not have control of outcomes, but only the process.\u201d<\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">Here are two scenarios to think through the implications in real-world clinical practice:<\/span><br \/>\n<!--more--><\/p>\n<p><img fetchpriority=\"high\" decoding=\"async\" class=\"aligncenter size-full wp-image-1508\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Do-Not-Become-Attached-to-Outcomes.png\" alt=\"\" width=\"560\" height=\"315\" srcset=\"https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Do-Not-Become-Attached-to-Outcomes.png 560w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Do-Not-Become-Attached-to-Outcomes-300x169.png 300w\" sizes=\"(max-width: 560px) 100vw, 560px\" \/><\/p>\n<h1><span style=\"font-family: Helvetica;\">Scenario #1<\/span><\/h1>\n<p><span style=\"font-weight: 400;\">A therapist presented a case as \u201clack of progress\u201d after three sessions. (Hats off to this therapist and to you the reader who have been systematically monitoring outcomes with every client). We looked at her outcome rating scale (ORS). \u201cBut the client is not progressing efficiently as the rest\u2026\u201d Granted, she should continue to keep an eye on the progress, but the truth of the matter, her concern was coming from a place of anxiety about one\u2019s own performance, rather than the nature of a client\u2019s growth and development. She was worried that her boss would notice the lack of speedy gains in her outcomes with this client.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Her engagement level with this client started low on the Session Rating Scale (SRS is used at the end of every session. See the <\/span><a href=\"https:\/\/scott-d-miller-ph-d.myshopify.com\/collections\/fit-manuals\"><span style=\"font-weight: 400;\">ICCE FIT Manuals<\/span><\/a><span style=\"font-weight: 400;\">for a useful guide), and was incrementally picking up. This is a GOOD sign. An increase in engagement across time is more predictive of good outcomes than high engagement levels throughout.[1]<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Here\u2019s my suggestion to her: <\/span><\/p>\n<ol>\n<li><span style=\"font-weight: 400;\"> Use the measures to guide you, not paralyse you. What paralyses? Comparison. (that\u2019s why facebook news feeds are a good way to trigger feelings depression).<\/span><\/li>\n<li><span style=\"font-weight: 400;\"> Reflect on the following: What are the things that are within your control as the therapist? What are the things that you are doing that facilitated an improvement in the engagement level. Once identified, continue to do so;<\/span><\/li>\n<li><span style=\"font-weight: 400;\"> Continue to monitor progress. Develop a rough algorithm for all of your clinical work, or a simple decision tree to decide when to<\/span><\/li>\n<li><span style=\"font-weight: 400;\"> do something different, <\/span><\/li>\n<li><span style=\"font-weight: 400;\"> seek further consult, etc.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">For this particular therapist, we know from her outcome data that reliable improvement happens around the 4-6th session (consistent with the literature). <\/span><\/p>\n<p><span style=\"font-family: Helvetica;\">&#8230;<\/span><\/p>\n<blockquote>\n<p style=\"text-align: center;\"><span style=\"font-family: 'Times New Roman', Times; font-size: 18pt;\"><strong>One of the ways feedback mechanism can backfire is to COMPARE with others. We must focus on the things that are influenceable and predictive of good outcomes, and be very clear on what we do not have control of.<\/strong><\/span><\/p>\n<\/blockquote>\n<p><span style=\"font-weight: 400;\">In this instance, this therapist was doing a fine job heightening the engagement levels, though the outcomes were \u201cnot as efficient\u201d as she had hoped for. We must not suffer fools in comparison. Instead, decision making must be paired with DATA + Clinical Knowledge, and not outsourced simply to an outcome measure alone.<\/span><\/p>\n<p><span style=\"font-family: Helvetica;\"><img decoding=\"async\" class=\"aligncenter wp-image-1506 size-large\" src=\"http:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions-1024x278.jpg\" alt=\"\" width=\"676\" height=\"184\" srcset=\"https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions-1024x278.jpg 1024w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions-600x163.jpg 600w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions-300x81.jpg 300w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions-768x208.jpg 768w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions-676x183.jpg 676w, https:\/\/darylchow.com\/frontiers\/wp-content\/uploads\/2017\/12\/Data-intuition-better-decisions.jpg 1301w\" sizes=\"(max-width: 676px) 100vw, 676px\" \/><\/span><\/p>\n<h1><span style=\"font-family: Helvetica;\">Scenario #2<\/span><\/h1>\n<p><span style=\"font-weight: 400;\">In another consult, the same therapist presented another case. This time, she shared one that she was happy with the progress after two sessions. There was indeed a big improvement on the outcome measure (i.e., ORS) just after the first session. However, the alliance ratings on the SRS has declined. \u201cDid you get some feedback as to the drop in SRS?\u201d I asked. The therapist replied, \u201cNope. We ran out of time.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Here\u2019s my suggestion to her: <\/span><\/p>\n<ol>\n<li><span style=\"font-weight: 400;\"> Create a buffer: Leave a few mins at the end of the session. Devote the time and space to talk about the feedback. Even if there\u2019s no significant verbal feedback, that\u2019s fine. Communicate how important the client\u2019s perspective is. (And then use those few minutes to breathe before your next session).<\/span><\/li>\n<\/ol>\n<ol start=\"2\">\n<li><span style=\"font-weight: 400;\"> When there is a drop in the alliance ratings, ASK for more information (See this <\/span><a href=\"http:\/\/darylchow.com\/frontiers\/2016\/12\/30\/how-do-you-get-better-at-eliciting-feedback\/\"><span style=\"font-weight: 400;\">post<\/span><\/a><span style=\"font-weight: 400;\">). This is important. Your client is telling you something is amiss or not as on-track as before. You might not find out if you say, \u201cI\u2019d get the feedback in the next session\u201d because<\/span><\/li>\n<\/ol>\n<ul>\n<li><span style=\"font-weight: 400;\"> Your client might say \u201coh it\u2019s nothing\u201d because they don\u2019t remember the details, or <\/span><\/li>\n<li><span style=\"font-weight: 400;\"> They might not be there to tell you, because they\u2019ve dropped out of treatment.<\/span><\/li>\n<\/ul>\n<ol start=\"3\">\n<li><span style=\"font-weight: 400;\"> When there is an increase in the outcomes, figure out what your client is doing that helped them. Stay close to that which is working. Generalise and extrapolate.<\/span><\/li>\n<\/ol>\n<blockquote>\n<p style=\"text-align: center;\"><span style=\"font-size: 18pt;\"><em><span style=\"font-family: 'Times New Roman', Times;\"><strong>&#8220;When you get involved in too much of the things you don&#8217;t have control it affects your focus on things that you have control of.&#8221;\u00a0<\/strong><\/span><\/em><\/span><\/p>\n<p style=\"text-align: center;\"><span style=\"font-size: 14pt; font-family: 'Times New Roman', Times;\">~\u00a0<strong>Legendary basketball coach John Wooden\u00a0<\/strong><\/span><\/p>\n<\/blockquote>\n<p><span style=\"font-weight: 400;\">Here\u2019s a contradiction to grapple with:<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While I should not be attached to the outcome of an individual client, if my overall outcomes shows that I\u2019m not effective, I should care. It is naive to repeat \u201cwe cannot become attached to the outcome\u201d in light that we haven\u2019t been helpful to the people we are supposed to help. This is why clarity on what are some key benchmarks[2] are so important.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">I\u2019m not saying we shouldn\u2019t care about the outcomes. Actually, we must focus on the importance of client benefit. We must remember that good outcomes happen as a consequence of what we do in the process. Viktor Frankl reminds us, \u201cHappiness cannot be pursued, but ensued.\u201d [see related post on my other blog, Full Circles <\/span><a href=\"https:\/\/darylchow.com\/fullcircles\/2015\/07\/18\/the-dark-side-of-pursuing-happiness\/\"><span style=\"font-weight: 400;\">The Dark Side of Pursuing Happiness<\/span><\/a><span style=\"font-weight: 400;\">]<\/span><\/p>\n<p><span style=\"font-weight: 400;\">We must look at and work on making the conditions ripe for cultivating and nurturing our client\u2019s growth, as we first facilitate to transform their suffering into true joy for Life.
